Quality Health Network (QHN) is Colorado’s health information exchange (HIE) on the Western Slope. When this Chat was recorded, they were preparing to launch the Community Resource Network (CRN), a care coordination and close-the-loop referral initiative, coupled with a social information exchange. CRN is designed to integrate interventions for individuals by allowing providers to work in concert across the domains of health care, behavioral health, and social determinants of health. During this Chat we learn about the challenges of building CRN and how they anticipated it would impact the community.
Since this chat was recorded, CRN was awarded 2nd place among 114 competitors in the Robert Wood Johnson Foundation’s “Social Determinants of Health Innovation Challenge.”
